The third round of the National Study of Physician Organizations (NSPO3) collected extensive data from physician practices of all sizes. These data are being used to examine the relationship between the structures of physician practices and the extent to which they implement processes characteristic of the Chronic Care Model and the Patient-Centered Medical Home (PCMH). The focus is on patients with asthma, congestive heart failure, depression, and diabetes. Data will also be used by the Aligning Forces for Quality (AF4Q) study team in their ongoing evaluation of the impact of the AF4Q project in 17 communities across the U.S.
