ACTIVATE is studying the impact of patient activation and engagement (PAE) in two large accountable care organizations—Advocate Health Care in Chicago and HealthCare Partners in Los Angeles. In particular, the study takes advantage of a naturally occurring experiment to assess whether patients with diabetes and/or cardiovascular disease who receive care from practices who are advanced in PAE activities have better quality and outcomes of care than patients receiving care from less advanced practices. The investigators are examining changes over time in patient activation measures (PAM), clinical metrics such as blood pressure, and patient reported outcomes of care (PROs) such as physical and social functioning. They also are assessing how changes in PROs relate to the culture, leadership, team effectiveness, and relational coordination of primary care teams. The impact of feeding back early findings to patients and care team members is also being studied.
