California Accountable Communities for Health

The project begins with a literature review and an environmental scan of current data sharing practices, both within California and nationally, that would be applicable for an Accountable Community for Health (ACH) type collaborative. Using information gathered in the environmental scan and literature review, a toolkit will be developed that outlines and recommends options for data and information sharing for ACHs throughout California, regardless of their current capabilities. The toolkit will outline the spectrum of data and information needs of an ACH as well as sharing options that could and/or should be employed by an ACH to either create a data and information sharing capacity where none exists or improve upon and maximize a currently existing system to meet optimum standards.
