A Systematic Review of Tools and Measures to Support Patient Engagement in Safety Net Settings

This report details findings from a systematic review of peer-reviewed literature focused on patient engagement measures and tools used in adult primary care populations and from key informant interviews with relevant stakeholders. The report provides key insights and implications for funders and researchers involved in patient engagement work and for operational leaders and front-line staff in safety net settings.

Accountable Communities for Health Data-Sharing Toolkit

This toolkit, sponsored by the California Health and Human Services Agency, provides a resource to assist communities in California and beyond in working to share data across sectors for health improvement. The California Accountable Communities for Health (ACH) Initiative, which aims to improve the health of each community by linking health care and community prevention activities, requires complex and cross-sectional data- and information-sharing among community partners. The ACH Toolkit responds to the need for a resource to facilitate ACH collaboratives in California, providing the steps and processes required to engage in data-sharing across sectors.

A Resource Guide for California Accountable Communities for Health 

This resource guide reviews strategies that accountable communities for health might undertake to address three chronic conditions: asthma, cardiovascular disease, and diabetes. Strategies include clinical, community, clinical-community linkages, policy, and environmental interventions. Recommendations from leaders experienced in collective community health interventions are also included. The report was prepared in partnership with The California Endowment as part of California's State Innovation Plan, and initiative led by the Health and Human Services Agency to reduce regional and statewide healthcare expenditures, increase value-based payments, and demonstrate progress on Let's Get Healthy California Goals.
